At Sentinel, we have made thousands of custom night guards for customers with permanent retainers. As long as the bonded wire is securely attached and not loose in any way, it generally does not prevent you from taking a dental impression or wearing a properly fitted custom night guard.

Why a Permanent Retainer and Night Guard Can Work Together

A permanent retainer and a night guard serve two different purposes.

A permanent or bonded retainer is a thin wire attached behind selected teeth to help maintain their position after orthodontic treatment.

A night guard is a removable dental appliance designed primarily to protect the teeth and dental work from the effects of grinding and clenching.

Because the bonded retainer sits on the tongue-side surface of the teeth, it does not automatically interfere with a custom night guard.

The key is that the wire is secure and the dental record used to make the guard—whether an impression or digital scan—captures the relevant anatomy accurately.

What Is a Night Guard Designed to Do?

A custom night guard is designed to create a protective surface between the teeth and help reduce the wear and damage associated with nighttime grinding or clenching.

Depending on the individual, grinding and clenching can contribute to enamel wear, chipping, damaged dental work, jaw tension and morning discomfort.

Some people also experience improvement in symptoms associated with grinding or clenching, including morning headaches, jaw tension and TMJ-area discomfort.

Results vary from person to person, and a night guard is not a guaranteed treatment for headaches or TMJ disorders. However, over the years, many Sentinel customers have told us that wearing a properly fitted custom night guard significantly improved these symptoms for them.

Can You Take a Dental Impression With a Permanent Retainer?

Yes. If your permanent retainer is securely bonded and not loose, you can generally take a dental impression with it in place.

This is something our laboratory sees regularly. Sentinel has made thousands of custom night guards for customers who have permanent retainers.

The impression should clearly capture the teeth and the bonded-retainer area so our technicians can evaluate the anatomy and fabricate the appliance appropriately.

What If the Permanent Retainer Is Loose?

This is the important exception.

If the bonded wire is loose, broken, partially detached or moving away from the teeth, have it evaluated and repaired by your dentist or orthodontist before taking an impression or ordering a new night guard.

A loose bonded retainer may change position, and creating a new appliance around dental anatomy that is about to be altered or repaired is not ideal.

Once the permanent retainer is secure again, you can proceed with the night guard process.

Can You Use a Digital Scan Instead?

Yes. If your dentist or orthodontist already has a current professional digital scan of your teeth with the permanent retainer in place, check whether you can obtain that file before taking another physical impression.

Sentinel accepts qualifying STL and PLY digital dental scan files.

After ordering with the digital-scan option, you will receive a secure upload link. Our laboratory reviews the submitted scan before fabrication to make sure the file contains the anatomy needed for the appliance.

If the scan is usable, you may be able to skip the at-home impression altogether.

Does the Lab Need to Know You Have a Permanent Retainer?

Yes. Let the laboratory know that the bonded retainer is present.

Before fabrication, the important considerations include:

• the permanent retainer is securely bonded,
• the bonded-retainer area is clearly captured in the impression or scan,
• the submitted dental record contains the anatomy needed for fabrication,
• and the night guard can be fabricated without inappropriate pressure on the bonded wire.

Will a Night Guard Press Against the Permanent Retainer?

A properly fabricated custom night guard should account for the dental anatomy present when the impression or scan was taken.

If a new guard feels as though it is placing unusual pressure on the bonded wire, does not seat completely or causes persistent discomfort, do not force it into place.

Contact the provider or laboratory that made the appliance so the fit can be evaluated.

Permanent Retainer vs. Traditional Braces

This is an important distinction.

Having a permanent bonded retainer after orthodontic treatment is not the same as currently wearing traditional braces.

We do not recommend taking a standard Sentinel at-home dental impression while traditional brackets and wires are still attached to the teeth.

Brackets and orthodontic wires create additional contours and undercuts that can make impression material difficult to remove and can interfere with obtaining an appropriate dental record.

If you are currently undergoing active orthodontic treatment with braces and need protection for grinding or clenching, speak with your orthodontist about an appliance appropriate for your current treatment stage.

What If You Also Wear a Removable Retainer?

Some people with permanent bonded retainers are also prescribed removable retainers.

That creates a different situation because both the removable retainer and night guard may be intended for nighttime wear.

Do not simply stop wearing an orthodontic retainer because you are beginning to use a night guard.

If you need both orthodontic retention and grinding protection, discuss both goals with your dentist or orthodontist so your appliance plan accounts for each.

Can a Night Guard Protect Your Teeth If You Have a Permanent Retainer?

Yes. The presence of a bonded retainer does not eliminate the effects that grinding or clenching can have on the rest of the teeth and dental work.

A night guard and permanent retainer are addressing different concerns: the retainer helps maintain orthodontic tooth position, while the night guard is intended to protect against grinding and clenching forces.

For many people, those two goals can be addressed at the same time.

How to Order a Sentinel Night Guard With a Permanent Retainer

If your permanent retainer is securely bonded, you can begin with either a qualifying digital dental scan or Sentinel's at-home impression process.

If you already have a professional digital scan: ask your dentist or orthodontist whether they can provide an STL or PLY file. If the file contains the anatomy our laboratory needs, you may be able to skip the impression process.

If you do not have a usable digital scan: choose the at-home impression option. As long as the bonded retainer is secure, you should generally be just fine taking the impression with it in place.
