Boil-and-Bite vs. Custom Mouthguards for Sports: What’s the Difference?

Boil-and-bite and custom mouthguards can both help protect the teeth and oral tissues during sports. The difference is primarily in how they are fitted.

A boil-and-bite mouthguard starts as a mass-produced thermoplastic appliance that you soften in hot water and shape around your teeth at home. A custom sports mouthguard is fabricated from a physical impression or digital model of your individual dental anatomy.

That difference can affect retention, comfort, bulk, speech and how likely an athlete is to keep the guard in place during competition.

Quick answer: A properly fitted boil-and-bite mouthguard can be a reasonable and affordable form of athletic dental protection. Custom mouthguards offer greater control over fit, thickness, border shape and retention and tend to perform better in studies of comfort and wearability. The best mouthguard is one that is appropriate for the sport, fits securely and is actually worn.

What is a boil-and-bite mouthguard?

Boil-and-bite mouthguards are widely sold in sporting-goods stores, pharmacies and online.

According to the American Dental Association, these mouthguards are commonly made from heat-responsive thermoplastic materials such as EVA. The appliance is softened in hot water and then adapted around the teeth using the fingers, tongue, lips and biting pressure.

When the molding process goes well, a boil-and-bite guard can provide much better adaptation than a completely stock, ready-to-wear mouthguard.

The challenge is that the final fit depends heavily on the original guard design and how successfully it is molded at home.

What is a custom sports mouthguard?

A custom sports mouthguard begins with a model of your actual teeth.

That model may come from a physical dental impression or, when compatible, a digital intraoral scan. The guard is then formed and finished around the individual's dental anatomy rather than reshaped from a universal stock form.

This allows greater control over areas such as:

  • retention around the teeth
  • border length and shape
  • material distribution
  • overall thickness
  • smoothness around the lips and cheeks

1. Fit and retention matter

A sports mouthguard should remain securely positioned without requiring the athlete to constantly clench down to keep it in place.

Retention matters because an unstable mouthguard can interfere with speech, breathing and concentration and may move when the athlete needs it most.

This does not mean every boil-and-bite guard will fall out. Properly fitted mouth-formed guards can work well.

But custom fabrication gives the laboratory more control over the individual contours responsible for retention.

In a 2024 randomized clinical trial involving basketball players, the boil-and-bite group reported less favorable results in several areas, including stability, comfort, speaking, breathing and drinking, compared with athletes wearing custom-made mouthguards.

2. Comfort can determine whether the athlete actually wears it

Protective equipment only works when it is used.

A mouthguard that feels excessively bulky, triggers gagging or makes communication unusually difficult may spend more time in a gym bag than in the athlete's mouth.

Custom mouthguards can be trimmed and finished around the athlete's individual anatomy, which may improve comfort without eliminating the material needed for protection.

Research comparing custom and self-adapted mouthguards has generally found advantages for custom guards in subjective measures such as comfort, stability and the ability to speak or breathe.

That does not mean a custom mouthguard should be made as thin as possible. Comfort and protective design have to be balanced.

3. More thickness is not automatically better

The older version of this article suggested that increasing thickness automatically creates more protection.

It is more accurate to say that thickness is one design variable among several.

The appropriate design depends on factors such as:

  • the sport
  • level and type of contact
  • the athlete's age
  • dental anatomy
  • other protective equipment
  • comfort and ability to keep the guard in place

A thicker appliance may offer useful material for high-contact situations, but unnecessary bulk can also interfere with comfort, speech or breathing.

The goal is an appropriately designed mouthguard—not simply the thickest one available.

4. Do not assume boil-and-bite materials are unsafe

This is another important correction from the previous article.

There is no good basis for claiming that a mouthguard is unsafe simply because it is mass-produced or manufactured outside the United States.

The ADA notes that boil-and-bite mouthguards commonly use thermoplastic copolymers such as EVA. Some OTC athletic mouthguards also qualify for the ADA Seal of Acceptance, which requires evidence addressing material safety, preparation and oral-tissue safety.

If material composition matters to you, check the manufacturer's actual specifications rather than assuming that “custom” automatically means safe and “store-bought” means unsafe.

Does a custom mouthguard protect better than a boil-and-bite guard?

Mouthguards as a category have strong support for reducing dental and orofacial injuries in sports.

The American Academy of Pediatric Dentistry recommends properly fitted mouthguards for organized sports with a risk of orofacial injury, while the ADA recognizes both custom-fit and properly fitted boil-and-bite designs as potential options.

Custom guards provide greater control over fit and design and have shown advantages in comfort and stability studies.

However, it would be too absolute to say that every custom guard provides more protection than every correctly fitted boil-and-bite guard in every sport and every impact.

The practical distinction: Custom fabrication gives you more control over the variables that help make a sports mouthguard secure, wearable and appropriately designed. A correctly fitted boil-and-bite guard remains a legitimate option—especially when budget or immediate availability matters.

Do sports mouthguards prevent concussions?

Mouthguards should primarily be discussed as protection against dental and orofacial injury.

Research on whether mouthguards reduce concussion incidence is much less consistent.

The American Dental Association describes the concussion evidence as mixed and notes that additional research is needed.

A sports mouthguard should therefore not be marketed as brain protection or used as a substitute for helmets, rule enforcement, proper technique, concussion protocols or appropriate medical evaluation after a head impact.

If an athlete has possible concussion symptoms after an impact, the mouthguard they were wearing does not change the need to follow the sport's concussion evaluation and return-to-play procedures.

Can a boil-and-bite mouthguard be good enough?

Yes.

A well-fitted boil-and-bite guard may be a sensible choice when:

  • budget is a major consideration
  • a guard is needed immediately
  • the athlete can mold it successfully
  • it remains securely in position
  • speech and breathing remain manageable
  • it is appropriate for the athlete's sport

The ADA specifically recognizes properly fitted boil-and-bite mouthguards as an option in appropriate circumstances.

That is why we would not tell an athlete to throw away a properly fitted boil-and-bite guard simply because it was inexpensive.

When does a custom sports mouthguard make more sense?

Custom fabrication becomes particularly attractive when:

  • the athlete repeatedly struggles with loose retail guards
  • bulk interferes with communication or breathing
  • the athlete participates frequently or competitively
  • a more individualized thickness or design is desired
  • previous mouthguards have been uncomfortable or unstable
  • the athlete wants a guard fabricated specifically around their teeth

Athletes with braces, significant dental work, missing teeth or unusual dental anatomy should consider professional dental guidance when selecting athletic protection.

What about boxing and MMA?

Combat sports place particularly high demands on a mouthguard because the athlete expects direct contact to the face and jaw.

Secure retention and the ability to breathe comfortably while exerting yourself become especially important.

A boil-and-bite guard can still be used if it is appropriate for the sport and fits well, but athletes who struggle with stability, bulk or communication may benefit from individualized fabrication.

Custom does not necessarily mean “made by your dentist”

A dentist can prescribe and coordinate fabrication of a custom sports mouthguard, but custom fabrication is not limited to chairside dental-office ordering.

Dental laboratories can also create custom athletic guards from accepted impressions or digital scans.

Frequently asked questions

Is a boil-and-bite mouthguard better than no mouthguard?

For sports in which mouthguard use is appropriate, a properly fitted boil-and-bite guard can provide meaningful dental and orofacial protection. Follow the manufacturer's fitting instructions carefully.

Why does my boil-and-bite mouthguard keep falling out?

Poor retention may result from an unsuccessful molding process, an appliance that does not match your arch well or distortion during fitting. If the guard requires constant clenching to stay in place, remolding according to the manufacturer's instructions or choosing another design may be appropriate.

Are custom sports mouthguards more comfortable?

They often can be. Clinical studies have found advantages for custom guards in several measures of comfort, stability, speech and breathing, although individual experiences vary.

Are boil-and-bite mouthguards made from bad plastic?

Not inherently. Many are made from thermoplastic materials such as EVA. Check the manufacturer's material information and relevant safety certifications rather than judging safety solely by whether the product is OTC or custom.

Can I use my sports mouthguard as a night guard?

Sports mouthguards and bruxism night guards are designed for different kinds of forces and different patterns of wear. An athletic mouthguard should not automatically be treated as an interchangeable substitute for a dedicated nighttime appliance.

Can a mouthguard prevent a concussion?

Mouthguards clearly play a role in protecting the teeth and oral structures, but evidence that they prevent concussions is mixed. They should not be presented as guaranteed protection against brain injury.

The bottom line

A boil-and-bite mouthguard is not automatically a bad mouthguard.

It offers inexpensive, accessible athletic protection and can be a reasonable choice when it is molded successfully and remains securely in place.

Custom fabrication costs more because the guard is created around an individual's dental anatomy and can be refined for retention, border shape, thickness and comfort.

For frequent athletes or anyone who repeatedly struggles with loose, bulky or uncomfortable retail guards, that additional control can make a meaningful difference.
